Have you also tried disconnecting the monitor from the PC and accessing the menu/running self-tests without an external signal present? Note: a monitor may or may not have these capabilities. Sometimes they are undocumented and not described in the user's manual.

For example, a Planar monitor I have, has a self-test that isn't in the manual.
It is activated like this:

Power-up the monitor

Simultaneously press the
Left OSD adjust control button (1st from leftmost position)
Right OSD function select button (4th from leftmost position)

The display enters into a continuous loop displaying
All
white screen
black screen
red screen
green screen
blue screen

That simple test lets me check for stuck or damaged pixels, improper driver scanning of the screen (so I can blame or rule-out video card signal and memory when "funny patterns", stripes, dots, sync problems, etc) are displayed, and problems that might be over-heating related.

You can "play" with various combinations and sequences of the buttons to see if you have simliar undocumented features. Sometimes a self-test button sequence has to be pressed at the same time you press the power-on button.

if you set your video mode to something conservative does the problem go away?

try 1024x768 at 60 Hz
